WHAT IS A UMID CARD?
A Unified Multipurpose Identification (UMID) card is an ID for four (4) government agencies in the Philippines. These agencies include Social Security System (SSS), Government Service Insurance System, PHILHEALTH, and PAG-IBIG. It's a great innovation so that people wouldn't have to get all four or three of this cards, but instead have one that serves all four purposes.
You can apply for this ID in all of the agencies included in the card. I applied mine through SSS because I was also claiming for my Maternity Benefits. So here's the steps to apply for a UMID Card:
STEP 1: Get an application form at any SSS branch. For initial enrolment, meaning you have no old SSS ID yet, this ID is for FREE. For those, for card replacement, a fee of PHP300.00 should be paid. This fee can be paid in SSS branch itself or in all banks and bayad center. Attach old SSS ID with the form as this will be surrender upon applying for a new one.
In my case, I lost my old SSS ID so I had to present an affidavit of loss and pay the fee.

STEP 2: Prepare valid ID/s.
You will need to present identification documents either primary or secondary documents.
Primary documents include:
Driver's license
Passport
Professional Regulation Commission (PRC) card
Seaman's book
If you do not have any of this you will need to present two (2) valid secondary documents:
Postal ID
School or Company ID
Taxpayer's Identification Number (TIN) Card
Membership Card issued by private companies
Overseas Worker Welfare Administration Card
Senior Citizen Card
Voter's Identification Card
ATM - with cardholders name, or with certification from the bank, if without name
Credit Card
Fisherman's card issued by the BFAR
GSIS Card
Health or Medical Card
ID CARD issued by LGUs
ID card issued by professional association recognized by PRC
Birth Certificate
Baptismal Certificate
Marriage Contract
NBI Clearance
PAG-IBIG Member's Data Form
Permit to Carry Firearms
PHIC Member's Data Form
Police clearance
Seafarer's Registration Certificate
Transcript of Records
Alien Certificate of Registration
Bank Account of Passbook
Certificate from: Office of the Southern/Northern Cultural Communities, Office of Muslim Affairs
Certificate of Licensure
Certificate of Naturalization from the BOI
Life Insurance Policy
Birth/baptismal certificate of children or its equivalent
STEP 3: Fall in line in the screening counter and wait to be verified.
STEP 4: Line for your Biometrics Scan at the Data Capture Counter
STEP 5: Get the claim stub and wait for your UMID to be delivered at home.
It is really that simple to get a UMID card at any SSS branch, but here are some tips I'll share to you to make it a lot easier.
TIP 1: Prepare needed documents ahead of time. Don't go with incomplete requirements. It wastes effort, money and time. Be sure that you have your valid IDs and money just in case you will need it.
TIP 2: The early bird catches the worm. Go there early to avoid the long lines. If you are first in, then you are first out.
TIP 3: Bring a lot of patience. Although it was written in SSS Pasay branch that they do not have lunch break, THEY ACTUALLY HAVE. They just do not stop people from coming in the office but they have an hour before they will call the next number. So make sure if you come around 9 or 10 or later, that you have eaten or brought something to eat.
TIP 4: Don't be shy to ask questions. This one I had to learn to always do, because I often wait for my turn on cue and find out either I'm on the wrong line or I have incomplete requirements. The staff are there to answer your questions, as long as they are asked politely.
So there you go, those are the basic about UMID card and will update you as soon as I get my card delivered.
Hi! Do I still need to bring my original SSS E-1 form? I also lost it (together with my ID)... I read in another article that it would take me 1 month to have it replaced. Thanks!

ReplyDeleteYou just have to wait for it to be delivered to you. Then you can use that already as government ID anywhere.
DeleteGusto ko rin magkameron ng ganitong I.D. Hirap din kasi most of the time ngayon ee puro valid IDs na lang hinihingi nila sa mga transaction, I mean mga government IDs, ganun. Minsan pa pag humingi dalawa! Sa ngayon, passport lang ang meron ako na valid ID. Sana magkameron ako ng time para makapag-apply nito.
